What to Expect

We do ask that you arrive fifteen minutes before your appointment time in order to copy your insurance cards and review your registration forms.

This appointment is a consultation only and is meant to provide you with information to make an informed decision concerning your treatment options. We encourage you to bring another person with you to your first appointment.  You can expect to spend sixty to ninety minutes at our office which could include the following:

•  consultation with your physician                           
•  schedule further diagnostic testing if needed
•  labwork
•  patient education presentation
•  meeting with our financial counselor to discuss insurance and answer your billing questions 

Forms

We do require some registration forms that will need to be completed. You can download and print them at your convenience.  Please complete these forms and bring them with you to your appointment.  This will expedite the registration process.
